Support at Home

A NEW WAY TO RECEIVE AGED CARE

Through the Support at Home Program, BCR Communities can help you stay independent, comfortable and connected in your own home.

You will have one dedicated Care Partner and a small, familiar care team who really know you - no call centre queues, no repeating your story.

With local staff, an experienced clinical team, and 35 years of community care, BCR provides award-winning support that reinvests back into the Shoalhaven and Illawarra.

Support that works for you!

Support at Home is the Australian Government's aged care program designed to help older people remain independent at home for longer.

But getting the right care isn't just about choosing services from a list.

At BCR Communities, we start by getting to know you -  your routines, your health, what's important to you and what you'd like to keep doing independently. Your dedicated Care Partner then works with you and your family to coordinate the right mix of support.

Steps to Starting Support at Home

1.

Contact My Aged Care.
 

Get in touch with My Aged Care to register and arrange an assessment to understand your needs and what support you may be eligible for.

2.

Have your Assessment.

An assessor will talk with you about how you’re managing at home, what’s important to you and the areas where you may need some extra support.

3.

Choose your provider.

Once you’ve been approved for Support at Home, you can choose a provider like BCR Communities to deliver and co-ordinate your care.

4.

Plan your support.

Your BCR Care Partner will get to know you, your goals and your preferences, and work with you to organise the services and support that are right for you.
