Supporting your choice to remain at home, with compassionate care centred around you and your family.

When you or someone you love is approaching the end of life, having the right support can make it possible to spend this important time at home.

The End-of-Life Pathway, funded by the Australian Government as part of Support at Home, provides additional care and support for eligible older people who have three months or less to live and wish to remain in their own home.

At BCR Communities, our compassionate team will work with you, your family and your healthcare providers to coordinate care that reflects your individual needs, wishes and preferences.

Care and support when it matters most

The End-of-Life Pathway provides additional short-term funding to help you receive the care and practical support you need to remain safely and comfortably at home.

Your BCR Care Partner will take the time to understand what is important to you and your family and help co-ordinate your services alongside your medical team and any palliative care services you may already receive.

The pathway is designed to complement, rather than replace, specialist palliative care provided through state and territory health services.

Steps to Starting the End-of-Life Pathway

1.

Talk to your doctor or care practitioner.
 

Your doctor or nurse practitioner will discuss your circumstances with you and complete the required End-of-Life Pathway form if you meet the medical eligibility requirements.

2.

Complete an aged care assessment.
 

You will need an assessment through My Aged Care. If you are already receiving Support at Home services, an urgent Support Plan Review can be requested.

3.

Develop your care plan
 

Once you are approved, your BCR Care Partner will work with you, your family and the people involved in your care to develop a plan based on your needs and preferences.

4.

Receive support at home.
 

We will coordinate your approved services and work with your healthcare and palliative care teams to help you remain at home with the support you need.
